Financial Policy

If you do not see your insurance company listed below, please contact our office and speak to our Billing Manager. We do not want to let financial problems prevent you from getting necessary medical care and we will do all we can to work out an equitable payment plan. Co-pays are expected at the office visits dictated by your insurance company and we accept check, Visa and Mastercard for your convenience.

We have trained personnel and a well equipped office with state-of-the art medical equipment to provide the highest standards of treatment in a pleasant, efficient and friendly atmosphere.

The following is a list of forms that will need to be completed at your first visit along with a list of insurance companies that we presently participate with.
